Independent Contractor Acknowledge Form

Replacement of an existing 45' span prestressed concrete box beam bridge on Steamtown Road over the west branch of Alum Creek with a new 71'-6" three-span continuous concrete slab bridge on capped pile foundations and approximately 400 feet of roadway approach reconstruction. Total project length is 0.12 miles.

This is used to perform construction of curbs, curb ramps and walks at various locations within Delaware County that are needing replaced on an individual basis. There is no set quantity for this program.

This contract is an agreement to perform installation of pavement markings at various locations within Delaware County when requested by the owner.

This contract will be used by the Delaware Soil and Water Conservation District for agricultural maintenance. This work will include tile repair, swale reconditioning, and ditch cleaning.

Replacement of the existing steel pony-truss bridge over Fulton Creek with a three-span continuous concrete slab bridge with minimal roadway approach reconstruction. Total project length = 0.08 miles.
