CITY OF LAKE CHARLES, LOUISIANA UNIVERSITY AREA – LARGE DIAMETER SEWER REHABILITATION CAPITAL PROJECT NUMBER CP3251 I. Scope of Services: Based on review of the CCTV data of the large diameter cleaning and CCTV project within the University area, the assessed pipelines have been designated as critically deficient and priority one deficient. Following review of the assessment the following scope of work has been developed. Base Bid: A. There is approximately 9,315 LF of 10-inch through 12-inch gravity sewer pipeline to be rehabilitated as part of this project. Rehabilitation methods include point repairs, pipe bursting, cured-in-place lining, and manhole lining. Also included within this contract is the reinstating of approximately 45 active service laterals and cured-in-place lining of approximately 60 active services. Alternate No.1: A. There is approximately 451 vertical feet of cleaning, repairing, and lining of new manholes. Alternate No.2 A. There is approximately 2,028 LF of 8-inch through 12-inch gravity sewer pipeline to be rehabilitated as part of this project. Rehabilitation methods include point repairs, pipe bursting, cured-in-place lining, and manhole lining. Also included within this contract is the reinstating of approximately 11 active service laterals and cured-in-place lining of approximately 6 active services.

HL-16 Downtown Lakefront Electrical City of Lake Charles, LA ~ City of Lake Charles project, ‘HL-16 Downtown Lakefront Electrical’ consist of repair and/or replace electrical components of the downtown / lakefront streetscape identified area lighting as outlined on the contract documents, especially luminaires and light poles. Address specific damage repairs due to the high winds and heavy rain presented by Hurricane Laura, including surge suppression and electrical system distribution products which were compromised.

City of Lake Charles, Louisiana City Hall Roof Repairs (HL-SOV-053) Hurricane Laura Disaster Recovery (DR-4559-LA) ~ I. Scope of Services: Design of roof repairs, in response to damage incurred as a result of Hurricane Laura (DR-4559-LA), to return the roofing system to pre-storm conditions and to upgrade these existing systems to coincide with newly applicable codes, standards, and regulations as imposed by governmental bodies with the appropriate jurisdictional authority. This Work shall include the following: 1. Removal and replacement of the modified bitumen roof assembly 2. Removal and replacement of damaged HVAC units and other damage mechanical accessories 3. Repair and recommissioning of damaged HVAC units which are not deemed to be beyond repair (if applicable) 4. Removal and replacement of the communication tower and its appurtenances 5. Repair of electrical racks, electrical panels, conduit and other associated appurtenances 6. Removal of skid mounted communication building and its appurtenances and removal of steel skid 7. Repair or removal and replacement of all other damages related to Hurricane Laura

Capital Project No. HL-54 Storm Repairs - Old City Hall Interiors City of Lake Charles, LA ~ Work involves removal and reapplication of plaster off of wood lath backing at ceiling and wall locations throughout. Repair of damaged gypsum drywall assemblies in other locations. Sanding and refinishing of existing wood floor. New paint and trim stained throughout. Miscellaneous repairs to plaster walls. Salvage, repair, and reinstallation of door hardware and fixtures as necessary.

Rerouting of influent sewer mains. Installation of 6 ft. hurricane fence with green privacy slats and construction of concrete access drive. • Sowela Lift Station - Complete rehabilitation to include new pumps, rails, electrical service, controls, and coating of the wet well. Rerouting of influent sewer mains. Demolition of abandoned concrete structures. Fencing to be installed by Sowela building contractor.
