City of Lake Charles, Louisiana Muirfield Drive Drainage-Phase II Project No. 2025-748 Capital Project No. 3224 The general scope of work for Muirfield Drive Drainage Phase II consists of the installation of approximately 1400 linear feet of 36” diameter storm drainage pipe to augment the existing drainage system. This pipe installation begins on the west side of Lot 19 of Turnberry Row subdivision and continues to the west line and into Lot 5 of the Muirfield Village Phase One subdivision. This additional drainage system will be installed along and within the limits of the Lake Charles Country Club Golf Course boundary.

Project No. 2023-726 Capital Project No. 3019 Sale Road Bridge Repair City of Lake Charles, LA The general scope of work for this project is the repair of the Sale Road bridge over Lateral L-2A of Contraband Bayou. This bridge exhibited various defects shortly after it was constructed in 2014. These defects included road pavement settlement, broken tie-rods, drainage pipe/box failures, etc. This project’s repair scope includes: • Construction of a new concrete deadman tie-rod anchor assembly to restore support to the sheet pile wall; • Replace failed drainage concrete pipe and boxes; • Removal and replacement of reinforced concrete pavement; • The associated traffic management, including detour signage.

Project No. 2019-685 Capital Project No. 3364 2019 Wastewater Pipe Bursting City of Lake Charles, LA The general scope of work is to rehabilitate a total of approximately 6,446 linear feet of existing 8-inch sewer main by pipe bursting at the six (6) project sites listed above. The pipe bursting process is to include 8-inch DR-17 HDPE pipe, all insertion pits, full circle clamps, and tie-ins to manholes. All sewer services shall be reconnected to the main. Also included in the scope is the rehabilitation of approximately 20 manholes and the installation of two (2) new 4-foot diameter fiberglass manholes. The scope includes a video inspection of each project site. Associated incidental work includes the removal and replacement of approximately 300 square yards of pavement, hydroseeding, and video inspection.

Division Street, Hodges Street, and Pujo Street Drainage Rehabilitation CP 3337 The work for this project includes, but not limited to, sawcutting and removal of concrete and/or asphalt pavement to expose and repair collapsed, blocked, or otherwise failed sections of existing vitrified clay, concrete, metal, PVC, or combination of storm sewer as indicated by the project plans for portions of N. Division, Hodges, and Pujo Streets. After completion of point repair activities, Contractor shall install Cured In-Place Pipe in continuous segments from designated structures listed on the plan sheets. Roadway surfaces damaged during the course of point repair construction will be repaired using like materials to return the roadway surface to a condition that meets or exceeds its condition prior to beginning construction.

The Work includes exterior cleaning of building, re-pointing all existing masonry walls, painting of all window trim and exterior “non-masonry” materials and all exterior surfaces that have been previously painted including stairs, exterior doors, etc.. Replacement of all gutters and downspouts including flashing at scuppers and repair of roofing at parapet walls, collector heads, and coping. 2. Scope of Services: The City of Lake Charles desires to replace the existing 3.0 MG steel at the Southwest Water Treatment Plant which is being demolished due to structural failures. In place the existing 3.0 MG GST the City would like to construct a new 2.0 MG concrete ground storage tank to expand the finished water capacity available on site. This Work will include but is not limited to the following: 1. Demolition of the remaining concrete ring wall foundation 2. Excavation of existing subgrade and installation of select fill or granular materials in as defined by the geotechnical investigation. 3. Construction of a new 2.0 MG AWWA D110 Type III concrete ground storage tank. 4. Installation of fill and draw piping to and from the ground storage tank 5. Installation of a remote level indicator and associated instrumentation and controls for tank monitoring and plant control. 6.
